Zepbound Eligibility: Who Can Take It
Zepbound is a prescription medication with specific eligibility criteria. Not everyone who wants to lose weight qualifies for treatment. Understanding these requirements helps you determine if Zepbound might be appropriate before seeking a consultation.
Basic Eligibility Criteria
According to FDA guidelines, Zepbound is indicated for adults who meet one of these two conditions:
Condition 1: Obesity
BMI of 30 kg/m² or greater
Condition 2: Overweight with Comorbidity
BMI of 27 kg/m² or greater PLUS at least one weight-related health condition
Calculating Your BMI
BMI (Body Mass Index) is calculated by dividing your weight in kilograms by your height in meters squared:
BMI = Weight (kg) ÷ Height (m)²
For example, a person weighing 85kg who is 1.7m tall:
85 ÷ (1.7 × 1.7) = 85 ÷ 2.89 = 29.4 BMI
BMI categories for adults:
| BMI Range | Classification | Zepbound Eligible? |
|---|---|---|
| Below 18.5 | Underweight | No |
| 18.5 - 24.9 | Normal weight | No |
| 25.0 - 26.9 | Overweight | No |
| 27.0 - 29.9 | Overweight | Yes, if comorbidity present |
| 30.0 and above | Obese | Yes |
Qualifying Weight-Related Conditions
If your BMI falls between 27 and 30, you need documentation of at least one of these conditions to qualify:
- Type 2 diabetes: Elevated blood sugar levels requiring management
- Hypertension: Blood pressure consistently above 130/80 mmHg
- Dyslipidemia: Abnormal cholesterol or triglyceride levels
- Obstructive sleep apnea: Breathing interruptions during sleep, often diagnosed by sleep study
- Cardiovascular disease: History of heart attack, stroke, or established heart disease
- Non-alcoholic fatty liver disease: Fat accumulation in the liver
- Polycystic ovary syndrome (PCOS): For women, often associated with insulin resistance
Many of these conditions improve significantly with weight loss, which is part of why Zepbound may be prescribed for patients with lower BMIs who have these health issues.
Absolute Contraindications
Certain conditions mean you should NOT take Zepbound under any circumstances:
Do Not Take Zepbound If You Have:
- Personal history of medullary thyroid carcinoma (MTC)
- Family history of MTC
- Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- Known hypersensitivity to tirzepatide or any component of Zepbound
These contraindications exist because GLP-1 medications have been associated with thyroid C-cell tumors in rodent studies. While this has not been confirmed in humans, the risk cannot be ruled out for people already predisposed to thyroid cancer.
Conditions Requiring Careful Evaluation
Some conditions do not automatically disqualify you but require thorough discussion with your healthcare provider:
History of Pancreatitis
GLP-1 medications have been associated with acute pancreatitis in rare cases. If you have had pancreatitis before, the risk-benefit calculation needs careful consideration.
Severe Gastrointestinal Disease
Conditions like gastroparesis (delayed stomach emptying), severe GERD, or inflammatory bowel disease may worsen on Zepbound. The medication slows gastric emptying, which can compound existing GI issues.
Gallbladder Disease
Rapid weight loss increases the risk of gallstones. If you have a history of gallbladder problems, your provider may recommend closer monitoring or preventive measures.
Diabetic Retinopathy
For patients with diabetes, rapid blood sugar improvements can temporarily worsen diabetic eye disease. Regular eye examinations are recommended.
Kidney Disease
While Zepbound is not primarily eliminated through the kidneys, dehydration from GI side effects can affect kidney function. Patients with existing kidney problems need careful monitoring.
Age Considerations
Zepbound is approved for adults 18 years and older. Special considerations apply at different ages:
- Under 18: Not approved. Safety and efficacy have not been established in pediatric patients.
- 65 and older: Approved but may experience more pronounced GI side effects. Slower titration may be appropriate.
Pregnancy and Breastfeeding
Zepbound is contraindicated during pregnancy and breastfeeding:
- Pregnancy: Stop Zepbound at least 2 months before planned conception. Weight loss during pregnancy is harmful to fetal development.
- Breastfeeding: Unknown if tirzepatide passes into breast milk. Not recommended during breastfeeding.
- Women of childbearing potential: Reliable contraception is recommended during treatment.
Note that Zepbound may increase fertility in women with PCOS or other weight-related fertility issues. If pregnancy is not desired, this makes contraception even more important.
Drug Interactions
Inform your healthcare provider about all medications you take. Zepbound can affect how other drugs work:
- Oral medications: Delayed gastric emptying can affect absorption timing. May need to adjust when you take other pills.
- Insulin and sulfonylureas: Risk of hypoglycemia increases. Doses often need reduction when starting Zepbound.
- Oral contraceptives: Absorption may be affected. Consider non-oral contraception or backup methods.
The Screening Process
Before starting Zepbound, expect your healthcare provider to:
- Review your complete medical history
- Calculate your BMI and document weight-related conditions
- Check for contraindications
- Order baseline laboratory tests (kidney function, liver function, blood sugar, lipids)
- Discuss realistic expectations and treatment goals
- Review the dosing schedule and injection technique
- Schedule follow-up appointments for monitoring
This thorough screening protects your safety and helps ensure treatment success.
Nigerian Context
Several factors specific to Nigeria may affect eligibility discussions:
- Access to diagnostics: If you suspect sleep apnea or fatty liver but have not been formally diagnosed, your provider can recommend appropriate testing.
- Family history: Thyroid cancer screening may be less accessible. Be thorough in reporting any family history of thyroid problems.
- Medication access: If you take medications for other conditions, ensure your provider knows exactly what you are using, including traditional remedies.
